RECOMMENDATION
Approve an Interlocal Agreement between Nueces County and Texas A&M Health Science Center for the creation of a Diabetes Care Team to improve health outcomes in a high-risk population in Nueces County and related matters for the period August 1, 2017 through September 30, 2019.
BACKGROUND
The City of Corpus Christi (“City”) and Texas A&M Health Science Center (“Contractor”) had entered into an agreement pursuant to City of Corpus Christi’s RFP No. BI-0020-14 seeking qualified Proposers to create a DIABETES CARE TEAM consisting of both Certified Diabetes Educators (CDEs) and Community Health Workers (CHWs) working through community Diabetes Self-Management Education/Support (DSME/S) programs to improve health outcomes via high quality DSME/S and care coordination in a high risk population. City-Contractor contract has since expired and now the Contractor wishes to continue to provide the same services at the same rates as in the City’s RFP No. BI-0020-14 under this Interlocal Agreement with the County of Nueces.
DISCUSSION
Contractor previously entered into a contract with the City to provide a DIABETES CARE TEAM in response to the City’s RFP NO. BI-0020-14, herein incorporated in the contract as Exhibit A. The Scope of work to be performed by Contractor is outlined in section 3 of Exhibit A. The City-Contractor contract has since expired. Contractor agrees to provide the services outlined in Section 3 of Exhibit A, and agrees to the same rates in Contractor’s response to city’s RFP No. BI-0020-14 and contract no. SA14-064, which is incorporated into this Interlocal Agreement as Exhibit B. These services and rates are for three years from the effective date of this Agreement. However, for years 2 and 3 of this Agreement, the number of patients may increase or decrease based upon funding from the Texas Health and Human Services Commission (HHSC) funding. Contractor understands and agrees that the number of patients to be seen may decrease or increase depending on funding by HHSC and hereby agrees to comply accordingly.

Contractor agrees to provide the services as set out in Exhibit A and B $1,120.00 per patient subject to funding by HHSC. The maximum amount payable per year are as follows:

a. Up to $300,000 through September 30, 2017 (identified as HHSC year 6);
b. Up to $410,000 -October 1, 2017 through September 30, 2018 (Identified as HHSC year 7);
c. Up to $410,000 October 1, 2018 through September 30, 2019 (Identified as HHSC year 8).
With a total amount of this entire contract of $1.250.000.

All invoices for payment and reports will be submitted to the Corpus Christi-Nueces County Public Health District that is the Contract Administrator for processing.

The Corpus Christi-Nueces County Public Health District recommends approval of the contract.
