4. B. 1.
Commissioners Court - Regular
Nurse Family Partnership
Dina Chavez, Health Department
Discuss and consider Amendment Five to a contract between the Corpus Christi-Nueces County Public Health District (CCNCPHD) and the Texas Health and Human Services Commission (HHSC) for the Texas Nurse-Family Partnership Program in the amount of $616,017.00 for the period of September 1, 2016 to August 31, 2017; and authorize the director of the Corpus Christi-Nueces County Public Health District to sign on behalf of Nueces County.
The contract introduces vulnerable first-time parents to caring maternal and child health nurses through a home visitation program that starts during 28 weeks of pregnancy. This nurse-led maternal health and home visitation program partners low-income, first-time moms with maternal and child health nurses. Resulting in a trusted relationship that gives these moms the support they need to have a healthy pregnancy, improve their child’s health and development, and become more economically self-sufficient.
The CCNCPHD is in receipt of Amendment No. 5 HHSC 529-12-0066-00005E with HHSC to provide funding for a nurse-led maternal health and home visitation program to improve pregnancy outcomes, child health and development, reduce child abuse and neglect, reduce domestic violence among participants and improve family economic self-sufficiency and stability. Under Amendment Five, HHSC will provide, for the term beginning September 1, 2016 and ending August 31, 2017, an additional $616,017.00 with a local match of $61,601.70 through an indirect De Minimis Cost Rate of 10% as authorized by HHSC Uniform Grant Guidance Sub-part E-Cost Principles Sections 200.412 through 200.415.
